Patient Records & Consumer Complaint Information

Peak Performance and Functional Chiropractic LLC provides the following information in accordance with Texas Health & Safety Code §181.105 regarding consumer access to health care records and complaint information.

This notice explains how a patient or consumer may:

1. Request health care records from our office;
2. Contact the applicable licensing or disciplinary authority for our clinic; and
3. File a consumer complaint as described under Texas Health & Safety Code §181.103.

1. How to Request Your Health Care Records

Patients may request a copy of their health care records from Peak Performance and Functional Chiropractic LLC.

When submitting a records request, please include the following information:

- Patient’s full name
- Patient’s date of birth
- Phone number and/or email address
- Dates of service or approximate treatment dates
- A description of the records being requested
- Where the records should be sent
- Patient signature or legally authorized representative signature, when required

Our office will process records requests in accordance with applicable federal and Texas law. Additional verification may be required before records are released in order to protect patient privacy.

2. How to Contact the Licensing or Disciplinary Authority

Peak Performance and Functional Chiropractic LLC is a chiropractic office. The applicable licensing and disciplinary authority for chiropractors in Texas is the Texas Board of Chiropractic Examiners.

3. How to File a Consumer Complaint

Consumers may also file a consumer complaint with the Texas Attorney General’s Office Consumer Protection Division.

Before filing a complaint, consumers should be prepared to provide information such as:

- The name of the business or individual involved
- The business address and contact information
- A detailed description of the concern
- Relevant dates, services, payments, or communications
- Any supporting documents, if available

Consumers should not include sensitive or confidential information in a complaint unless specifically required, such as Social Security numbers, financial account numbers, or unnecessary medical details.
